As there few/nil of these conditions/events in Paediatric sites in 1998, these problems were not asked for Paediatric/Adolescent patients except for Peripheral Neuropathy. The percentages in brackets are the All Forms and the Paediatric/Adolescent Forms missing data, for the one of these fields collected on all individuals. All others are Adult Forms only data.
