We got involved in the Health Care Home trials right at the beginning. It's very important for practices, before they start talking to patients about Health Care Homes, to be really clear what they have to offer. At the end of the day, the Health Care Home program is about patients. Some of the concerns raised include whether or not they're still able to access me as their doctor. This model helps us build on the servicethat we provide to them, not replace the face-to-face care that we provide to them. It allows us to be more proactive in their care and help them become more proactive in their care, and that's the beauty of the model, is that the work is actually divided up depending on the expertise of the actual staff member. So, I think as a practice you need to get together and you need to discuss what can we actually do, what can we do for our patients, what can we deliver, how are we going to change our model of care? Since we started Health Care Homes,the roles across the team have widened, not only for the nurses here, reception staff as well. It's allowed me to be more responsible for my patients. We have to upskill constantly and make sure that our staff are confident with the skills, and that also helps in the trust. As a Health Care Home, we're looking at that whole person. What are their needs that they don't even know that they need? We're looking at data, we're looking at things that we've actioned, and we're thinking this is what the evidence says, and this is what we need to do for our patient. What's worked for us is actually having a list of the Health Care Home enrolled patients, and so we then share that spreadsheet amongst the staff and each person has delegated roles and responsibilities on that spreadsheet. And so that ability to sort of get a bird's eye view of the cohort of patients enrolled and share that bird's eye view with the staff members is really important. Develop a work flow, develop a protocol, develop a script, do some in-house training, come to a shared understanding of what you're trying to do and why and how you're going to do it. The underpinning of all of this, of any delegation, is making sure that it's done in a safe and effective way and that patients' safety comes first and foremost.
